[Fredslist] Jury duty waiver for a disabled person

Folks,
My mom is 67, disabled, cannot stand or walk.
She's under the care of home health aides.
She received a summons/notice to appear for jury duty.
In the past, we were able to waive it online.
The city won't let us file for a waiver or exemption online.
How can I/we get a permanent waiver for her from jury duty?
