Hi Stephanie,
 
I am guessing here from years of experience, but having an NPI although
strongly recommended it is not required, unless that provider is planning to
enroll to be a Medicare provider. 
I have clients whom are receiving services from providers not having an NPI
number and most insurance carriers are making payments. With that said I
strongly recommend that your client apply for a minimum of a
personal/professional NPI number.  Adding a business NPI number will not
hurt either. These numbers are issued free of charge and without much
hassle.  I have done it for a few of my professional clients. 
Getting conflicting information from CMS and any other source is just the
way the entire healthcare industry dysfunctions.

Hi Gotham - 

I have a client who is a healthcare provider and just recently formed a PLLC
for her practice.  She received conflicting information about whether she is
required to obtain a Type 1 NPI (for individual health care providers) or
Type 2 NPI (for organizations).  I did some research and called up CMS
(centers for medicare and medicaid services) to confirm the requirements and
I received conflicting information as well.

Does anyone along Gotham know if a healthcare professional who is
incorporated is required to apply for a Type 1 NPI, a Type 2 NPI or both?
