Expanded Directions to Hazel House
The address of Hazel House is 80-45 Winchester Boulevard, Building 74,
Queens Village New York  11427. That address will take you to the south side
of Creedmoor Psychiatric Center which is between Union Turnpike and Hillside
Avenue.  The entrance to the campus is on Winchester Boulevard between
Hillside and Union (note that it is SOUTH of Union Tpke., even if GPS or
Google Maps says otherwise). There are three entrances on Winchester Blvd.
There is one for Lifeline, one for a housing complex called Country Pointe,
but the entrance to the south side of the campus is in the middle - take the
middle entrance.  (Just for reference, the North Side of the Campus has the
tall building owned by the State -- don't go there).
 
Once you get onto the campus, there is Building 60 right in front of you.
Veer toward your right and then just go straight past Building 60 till you
can't go any more. At the end of that road, make a left, and go down
approximately 3/10 of a mile -- on your left you will see Hazel House,
Building 74 (there's a small sign), on your left.  Make a left, and there is
parking in front of you.  This is adjacent to the side of Hazel House.
There is a lawn in front of Hazel House and a Gazebo.  The front entrance to
HH is adjacent to the lawn/gazebo.  We expect to have temporary HBA signage
placed for the meeting.
