[Fredslist] Credit card payment for golf.

We have set it up so that you can pay by credit card for the Golf outing 
.  However, after you pay, you must print the pdf file, fill it out and 
either mail or fax it to Dana Charlton at Fred's office. This is very 
important. If we don't have the proper information, we can't assign you 
a spot in a foursome. Fred's address is on the pdf. His fax number is
