[Fredslist] Termination of Parental Rights

Dear Gotham Family,

Does anyone have a model TPR agreement or an agreement whereby a parent
temporarily gives up custody of a minor child to go live with another
family member?

I have a client that I am representing on a pro bono basis and I am
looking for some guidance in this area.

Any assistance would be much appreciated
